Types of Appointments

Annual Physical

This is a preventive visit only! No new problems, old problems or refills. This is where we discuss cancer screenings, vaccinations, and other preventive care topics.

Chronic Conditions Check

This is an appointment that is not unplanned. It occurs every 1, 3, 6, 9, or 12 months, depending on the condition. This is where we order/review lab work and refill medications for that condition.

Please note there is a limitation on the number of conditions we can discuss based on the allotted time.

New Problem Visit

Urgent and non-urgent. This is a visit dedicated to a new problem.

Post-Hospital Visit

After being discharged from the hospital, here we review new medical conditions, new medications, any follow up labs and appointments.

Many appointments can now be virtual

Please note that not all appointments may be suitable for virtual visits.

If you request or book a virtual appointment, please be prepared with the following:

  • Home blood pressure readings
  • Current weight
  • Health card number
  • Medication names and doses
  • Preferred pharmacy information

If you need Dr. Doulaverakis to review any forms or photos (e.g., of a rash or visible concern), please email clear photos to patientcare@drchristina.ca before your appointment. Note: This email is not secure.

If an in-person appointment is preferred, please let our staff know.

What does OHIP not cover?

Prescription Renewals Without Appointments
Charge of $25 each
(For free renewals, please address the medication at your Chronic Conditions Check or check your email on how to subscribe to Block Fee to have these covered)

Missed Single Appointment
$40 charge

Missed Double Appointment
(Multiple issues, mental health, annual visit, diabetes visit): $100 charge

No-Show Policy
If no-show for 3 or more appointments, this may result in discharge from the practice.

Work or School Notes/Forms
Start at $35 and increase based on time/complexity

Driver’s Medical
$160

TB Skin Test and Reading
$50 per test

Other Services Not Covered by OHIP
Charge based on OMA rates

 

Non-Malignant Skin Procedures

Wart Treatment (Cryotherapy with Liquid Nitrogen)
$20 per session

Removal of Skin Tags
$40 per skin tag (maximum $200 per session if having multiple skin tags removed)

 

Additional Services

Transfer of Medical Records
Charges apply

Photocopying/Printing
$1 per page

Cosmetic Skin Procedures
Charges apply based on procedure

 

Payments accepted: Cash, Cheque payable to Dr Christina Doulaverakis, or e-transfer patientcare@drchristina.ca.
