Please keep your contact info updated with us at all times including phone #, mailing address, email and preferred pharmacy. This will help avoid many delays with prescriptions and referrals.

Ensure Email Attachment Access

Please make sure you have opened email attachments from our office in the past. If you have not been able to do this please call us immediately to ensure you are able to access our emails and attachments.

Specialist Referrals

If you have not heard back from a referral to a specialist or test in 2-4 weeks, please let us know.

Book The Correct Appointment Type

If you have more than two concerns, please inform the front desk when booking so that we can prepare for your visit and respect both your time and that of other scheduled patients.

Please Note:
Medical visits for new issues are generally limited to addressing a single concern. Additionally, some issues are not suitable for telemedicine—especially those that may require a physical exam, such as:

  • Chest pain
  • Shortness of breath
  • Dizziness
  • Headaches
  • Blurry vision

If you are experiencing a medical emergency, please call 9-1-1 immediately.

Disrespect & Harassment

Disrespect or harassment of any kind will not be tolerated and may be grounds for dismissal from this medical office.
